Reports your doctors respect and your patients read

15 ready-made designs, your letterhead, your logo, your signature, your colours — set once, and every report the lab prints follows. Download any of them below and open it on your own printer before you decide.

Every file below is a real report printed by SamLab, watermarked as a sample. The lab, patient and results in them are fictitious.

What should a laboratory report format contain?

A laboratory report must identify the patient and the specimen, state each test with its result, unit and reference range, flag values outside that range, and name the person who verified it alongside the laboratory and its address. SamLab ships multiple report designs and a designer for laboratories needing their own layout.

Clinical dashboards

Cards, charts and a history panel. The layouts that make a result look like it was read, not just printed.

Clinical Dashboard

Opens with the test name in a coloured band, then patient and sample cards, a differential pie beside the grid, and a trend graph with a dated history table. Closes on Summary / Interpretation / Recommendations and three signature blocks.

The one to show a referring doctor.

With a written summary

Every out-of-range value restated in plain words at the foot of the sheet, so nobody has to hunt the table for what changed.

Draw your own

Place every header field yourself on a canvas, and let the results flow underneath.

User Defined report format — page 1

User Defined

You place each patient and sample field yourself on the designer canvas. The results tables keep flowing underneath, so a long report can never overrun the layout you drew.

Matching a form you already print.

Same report, your identity

Set it once. Every report follows.

Upload the letterhead you already print on and SamLab scans it, finds where the artwork ends, and keeps the data inside the safe zone. Same for the logo and the authorised signature — cropped, cleaned of their white background, and synced to every branch of the lab.

Smaller type, more per page — sample report

Smaller type, more per page

Font size is a lab-wide setting; the whole report re-flows rather than shrinking a table into the margin.

On your own letterhead

Upload the lab's letterhead once; the report prints inside the detected safe zone and the generated header steps aside.

Black & white — sample report

Black & white

The same report on a mono laser — tints become greys that still separate, and the abnormal values stay legible.

Clinical Dashboard (black & white) — sample report

Clinical Dashboard (black & white)

The department band, the differential pie and the trend graph printed without a colour cartridge — the charts stay readable because the segments differ in tone, not only in hue.

SamLab Modern (black & white)

The card layout on a mono laser: the accreditation strip, the column accents and the status pills all survive as greys that still separate.

On pre-printed stationery

Paper already carries the letterhead, so the report leaves the top and bottom bands empty and prints only the data.

Report colour

Any colour you run

The accent drives the header band, the department rules and the table captions. Pick one for the lab, or let each department carry its own so a technician knows what came off the printer before reading a word.

Printed language

The report speaks to the patient

Column captions, the patient band and the footer print in the language the lab reports in. Right-to-left is a real layout, not a mirrored one — an Arabic patient name joins correctly and sits above its Latin transliteration.

Country packs also set the currency, the date order and the phone format on everything else the lab prints.
