The detail
Patient SMS notification — "your report is ready" — is on the roadmap and is not built. In Indian practice WhatsApp has largely displaced SMS for this, which is why it was built first, but SMS still reaches feature phones and patients who do not use WhatsApp.

Not today. It is on the roadmap. WhatsApp delivery is available now.
Will I need my own SMS gateway account?+
Almost certainly, along with DLT template registration, which is a TRAI requirement in India and applies to the sender regardless of software.
Is there a workaround?+
Labs that need SMS today generally use their existing gateway outside SamLab.
